MAO Questions for the experts

I know this board is the best place to ask technical questions.

I am planning on starting on Nardil. I pretty much have been on all the ADs and am ready for an MAO. My strange life-long depression has a weird component. (Very depressed in the AM and almost normal again by bed time - or at night !) I also have severe social anxiety, anhedonia, total lethargy (I am almost totally house-bound and only 27/male) Stimulants make me much more depressed even though I have Adult ADD -- I even went to a special school for kids with ADD and have had it since I was 3 and diagnosed at 4 or so. (not bipolar).I am pretty vegetative and my memory is getting worse and all that 'good' stuff. You get the idea.

I am very fearful of MAO but am very educated on these meds, as I have read a lot. I fear the Hypertensive crisis. I am a trained (non-practicing Paramedic) So this kinda makes me a little more worried. If my Doc oks it I am going to carry Procardia And thorazine in case.

My question is: Isnt it possible to go into shock (hypotensive) if we take the procardia or other Calcium channel blocker or alpha blocker, during the crisis.... which would be actually worse than hypertension because of the even more immiment possibility of becoming unconcious and brain dead?? I have seen it before!

I plan on using thorazine as a first line pill in the unhopeful event of the crisis... it seems it is much less likely to lower the blood pressure too low.... and possibly even impossible too become hypotensive on it, except if its taken I.V.

Any ideas here? Also is Compazine able to lower B.P. in a similar way to thorazine? Any experience?

Will a Benzodiazepine taken during a crisis help also. I know sedation in general can help get rid of the severe pain of hypertension (from sedation of the central nervous system) and also lower B.P. Will it it mitigate the effect of an overabundance of tyramine and its effect?

A non serotogenic benzodazapine can help somewhat in a hypertensive crisis.

The best thing to do is to buy a high quality blood pressure monitor. In the advent of a hypertensive crisis, you can take a portion of them medication and then check your blood pressure.

I know of a woman who's been on a MAOI for decades, and it's never come up.

Hi!

Compazine reduces blood pressure by the same mechanism as Thorazine but to a lesser extent. Thorazine would be a much better choice than Compazine.

Or anyone who can answer my slightly odd questions. I'd like to get into contact with people who are currently or past nardil user somehow....in a way that wouldnt be against chat rules... I am a little concerened and scared about starting....

Just be careful with foods and meds. I was on Parnate for almost 15 years and never had any hypertensive crisis's.

My MD never had a patient who had to go to an ER (30 years in practice).

Had some patients keep nifedipine on hand, just in case.

I have struggled with depression for the past 35 years and finally went on Prozac in 1990. However, I soon developed treatment resistance, finding many drugs ( SSRIs, Effexor, Serzone, TCAs, Wellbutrin) either intolerable or ineffective. I did have a successful therapeutic run of four years with Remeron. After the Remeron gave out, I went through 8 months of hell trying different combinations of medications, as well as ECT.

A little over a year ago, I started Nardil. Nardil has turned out to be a wonder drug for me, completely alleviating my anxiety and depression. I’m currently taking 45 mg a day. I have relatively few side effects: slight food craving, occasional low blood pressure, somewhat impaired ability to reach orgasm ( though that seems to be getting better now after four months at 45mg).

I found the only way to control weight gain and food cravings was to go on a fairly strict Atkins diet ( less than 20g of carbohydrate per day). I have been able to maintain constant weight now for over a year and I have no cravings at all for sweets. On this diet, I never feel hungry or deprived.

I have found the MAOI diet restrictions quite easy to follow. I have not had a hypertensive crisis, though I thought I had one once and took one Nifedipine. My blood pressure got fairly low and I felt pretty wiped out for a few hours.
