Anyone got psychotic from Mirapex (Pramipexole)?

Posted by rod on July 16, 2004, at 14:03:22

Hi all,

I will start Mirapex tomorrow...
Although I am not psychotic and have never been, my doctor is very concerned that I might get psychotic, severe psychotic. He also told me that you do not notice when you become psychotic. And the Likelyhood to develop psychosis due to Pramipexole is relatively high. So I am also a bit scared.

So, are there any people on this board who tried it and had a psychotic reaction? What do you think of this?

thank you
Roland

Hi Roland.

How high is high? Although I am aware of this as a possible side effect, I have never heard the phrase "relatively high" used to characterize it. I don't think it is so high as to be scared of.

From www.rxlist.com

"The events listed below occurred in less than 1% of the 1408 individuals exposed to MIRAPEX:

Nervous system: agitation, CNS stimulation, hyperkinesia, psychosis, convulsions. "

Of the few dozen people that I have seen use it on PB, I haven't seen anyone describe anything like psychosis.

Hi Scott,

Ok, you are right about this. I didnt look up the numbers before I posted. But now I am a little confused about psychosis vs. Hallucinations. Arent hallucinations also regarded as psychotic reaction? I thought one can have psychosis without hallucinations, but no hallucinations without psychosis. By saying psychosis I also meant hallucionations.

from rxlist.com - Warnings Precautions
"..hallucinations were observed in 9% (35 of 388) of patients receiving MIRAPEX, compared with 2.6% (6 of 235) of patients receiving placebo.."

And by saying relatively high, I meant compared to standard antidepressant treatment, where hallucinations/psychosis seem to be rare (also average of 1/1000, varying from drug to drug). I mean, I dont see any warnings about hallucinations in the prescription information from ADs like in the one from Mirapex.
And my doctor seems to be a bit overcautious, because once a patient jumped out of the window on pramipexole, because the devil was after him... Although this happened in a study for pramipexole for deficit syndrome, some years ago, and not for depression. Maybe this was just bad luck... Since then, he does not have a good feeling prescribing pramipexole (and I am the first one since then).

Anyways, I am hoping everything goes well on pramipexole.

thanks for your opinion and time
Roland

Roland, you are totally correct. The risk of psychotomimetic side effects is significantly higher with Mirapex than it is with standard antidepressants. I didn't mean to minimize your concerns about it. I apologize.

Is there anything in your history that you are particularly concerned with?

I think Mirapex is a better choice than Permax or Parlodel because it works on D3 receptors along with D2. In addition, it is purported to have neurotrophic and neuroprotective effects. But I'm sure you knew that already. :-)

Good luck! You deserve it.

Its a dopamine agonist approved for parkinson.
