I've known people who have gone above 60mg. Once I was on 20mg SR 4x daily = 80mg. That was many years ago.

I was thinking of taking the ER form, but heard its not as effective as the IR?

Studies have shown that the ER form lasts only *slightly* longer that the instant release form of Ritalin - that's why I was taking 20mg ER 4x/daily.
But now that Ritalin's patent is expired, other companies have made some good improvements in extended release forms. One of them is CONCERTA. Max dose is 56mg/day. They come in 18mg extended release tablets. I'd say they are MUCH MUCH better than Ritalin ER and IR. METADATE (sp?) is another - but I have not had an experience with it.

No sorry jerrympls, doesn't help me. Im in New Zealand and all we have is generic Methyphenidate and Ritalin 20mgSR and thats it! sad ha?. or Dexamphetamine 5mgs pills,(hardly ever used here) like australia (dexedrine in yr country) pills . You yanks have all the good meds. even script Methamphetamine, which New Zealand dropped round 1968 i believe.

20-30mgs total dose in 2-3x a day. Some Patients respond best to 40-60mgs total, in 2-3 x a day, (with doses above 60mgs a day not recommended)<manufactures prescribing guidelines>.

How are the doses usually spread out? I'm taking 20mg total. 10 in morning and 10 in mid afternoon. I don't really notice a difference when it comes to being able to think but I've noticed that at times I'm better able to focus. Unfortunatly, focusing, although a problem, is not really the main problem. I have real trouble sustaining a train of thought, thinking complexly, memory problems, word finding problems, etc;basically all the typical psuedo dementia symptoms but to a terrible extreme .Should I increase the dose, try something else,will no stimulant work if I'm still depressed? any thoughts?

I've thought about the same thing as far as will a stimulant work in a patient who is still displaying symptoms of depression. Are you currently taking an antidepressant? If not, then adding one seems prudent. Depression often co-exists with ADD/ADHD. It's common, from what I've read in the literature, that depression may resolve once a person begins taking a stimulant. If the depression does not resolve, then you probably have both conditions - ADD and depression. I would consult with your doctor.
