What to try next, please help!

Hi!

I´m a 25 yrs old male studying at a university, I have a relatively long history of pmeds. Currently on Remeron for depression and Klonopin for social anxiety. Remeron worked fine for me for several years but now it has quit working, I raised the dosage but it only helped for a month. It´s exam period next week and I have absolute no motivation to study, which is really frustrating because I´m really interrested in what I´m (supposed to be) studying.

I really need some advices, what med could help me out from this? I´ve read about Wellbutrin, Zoloft and Paxil but have no experience of them. Please help..

There are a number of things you can do. You've already mentioned adding a new antidepressant. Of the one's you've listed perhaps Wellbutrin would be your best choice since it tends to be energizing and may help you to concentrate.

The problem with adding a new antidepressant is that it can take several weeks to find out if it does anything good for you. Another avenue to explore is augmentation of your remeron with something more fast acting like ritalin, some form of amphetamine, or modafinil. If they are helpful for you you'll find out fairly quickly. Similarly you could try adding an AP like, zyprexa, risperdal, seroquel, or geodon. APs tend to be sedating, but will often work in less than a week, if they are going to work.

Just some suggestions.

I can empathize--a similar thing has happened to me recently. Wellbutrin may be worth a try. For me, it is somewhat energizing. You may feel a bit agitated or have trouble sleeping at first, but if you are like me, when I'm really down my problem is too much sleep and fatigue, so a short course of insomnia doesn't kill me. Especially if you have to stay up and study anyway.

I've tried nearly every AD, and Wellbutrin is one of the few I can tolerate. The side effects on others are too much for me, particularly the sexual side effects with the SSRIs (Prozac, Paxil, Zoloft, Celexa) and Effexor. But Wellbutrin isn't enough for me right now, so I am augmenting it with Adderall (a stimulant). So far it really seems to help. But since you are younger, I fear that if you asked for that or Ritalin, the doc might think you are trying to get it for abuse, not use (Adderall is a schedule II controlled substance). Provigil (modafinil) might work (if Adderall doesn't work I may ask for that one) and it is a schedule IV so its easier to get, but expensive.

You are doing the right thing by looking for a different approach that will work for you. Good luck.
