Effexor Sexual Dysfunction

My Doc says Serzone now causes Liver damage and has weaned
me off and onto Effexor @ 75mg/day. Now suffer
impotence & incontinence & only in mid-40's. Anyone
having this same result ? What was your solution?

There are many solutions to your difficulties but no concensus as to which pharmacotherapy works best. One just has to try them all to get the best "deal." Here are some possibilities:

Methylphenidate †
- 5-40 mg/d
- For SSRIs or venlafaxine
- Libido, arousal, orgasm

Ginkgo biloba extract †
- 180-240 mg/d, tid, divided doses
- Potential increased clotting time, possible flatulence
- Libido, arousal, orgasm

Amantadine
- 100 mg bid
- Caution in patients predisposed to psychosis
- Orgasm

Cyproheptadine
- 4-12 mg qhs
- MAOIs, TCAs, SSRIs, venlafaxine; watch for reemergence of depressive symptoms; sedating
- Orgasm

Buspirone †
- 30-60 mg/d, bid divided doses

- Libido, orgasm

Bupropion †
- 75-150 mg/d, qd or bid divided doses
- For SSRIs or venlafaxine, fluoxetine may raise bupropion levels; usual precautionary measures for bupropion
- Libido, arousal, orgasm

Mirtazapine †
- 15-45 mg/d
- For SSRIs, venlafaxine
- Orgasm

Sildenafil
- 50-100 mg/d
- Contraindicated with nitrates
- Libido, arousal, orgasm

Yohimbe †
- 5.4 mg tid
- Can be anxiogenic; ? safety with MAOIs
- Libido, arousal, orgasm

Here I go again on adrafinil, but I recently read how some users have said it improved their love life. I can't say one way or the other.

Jeez, I certainly hope your doc didn't take you off the Serzone if it was working for you and you had no evidence of liver dysfunction or a priori risk factors.

The chances of liver damage from Serzone are about 1 in 200,000. If all of the adults in Pittsburgh were taking Serzone, there might be one or two cases of liver damage. If the liver risk was so significant that patients should stop taking Serzone, the FDA would have removed it from the market -- especially since they're particularly sensitized to this issue right now given the Baycol and Rezulin situations.

I not only took Serzone for almost two years, but I took it with two medications it interacts with. I had several liver blood tests during that period (which is the prudent thing to do), and had no problems. Indeed, two enzymes which had been elevated BEFORE I took Serzone dropped well into the normal range.

My guess is that your doctor is a GP, not a psych.
Even if I guessed wrong, though, the mini-rant still holds.

Regardless, what's most important for you is getting better, and Effexor can be an excellent antidepressant. But if the Serzone was working well, and without sexual dysfunction or other side effects, the "liver scare" alone was not a good reason to change (unless, as I said, you have impaired liver function or associated risks).

But that has a liver-damage warning, too! The fact that doctors are over-panicking and taking people off Serzone convinces me that Cephalon will make no attempt to market adrafinil in the U.S. even though they have secured the rights by buying Lafon.

#### Rick,,, My Psychiatrist told me 6 days ago, to go OFF the Effexor, and he put me back on a lower dosage of Serzone. Guess what. No Effexor,, and No More Sexual Dysfunction, or Incontinence. Everything is back too normal in 6-7 days and I couldn't be happier. He and my GP, both said they had never heard of Effexor causing the above. I had liver blood work, Glucose blood work, and "clean" urinalysis (no infections)... Results - "All normal". In summary, "in my case anyway", Effexor apparently DOES/DID cause those drastic side effects. I concur with you that I should and will continue to have my liver enzymes tested now that I am back on Serzone, which is now reported to cause liver damage, in a small segment of the population. Thanks. MJB54 ##########
