I'm very fortunate:I have absolutely zero side effects. My psych doc has a lot of experience with MAOI's and prescribes them often. I concur with him that the tyramine/hypertensive crisis is way off the mark and way over exaggerated (as long as dose is correct for patient). The PDA (Physician's Desk Reference) literature about MAOI's role as such a bad actor and threat to life, that the vast majority of psych docs won't even mention it as an option for TRD (Treatment Resistant Depression). Such a shame.
I already know I'll catch a bunch of flak for my & my psych doc's opinion. All I know is that I went thru the gamet of trying a gazillion SSRI's, SSNI's, and even trycyclics, I was ready to give up on life. Until 1988 when I started on Nardil. It was the Gold Standard for TRD coupled with social phobia---until Pfizer f....ed it up in 2003.
Sorry I got so wordy - this is a subject close to my heart that is regularly denied needy patients because of over exaggerated risks. And yes critics, there have been some tyramine and hypertensive crisises in the literature. Nothing is absolutely perfect. But when nothing else works, are you willing to try something that may have a bit of risk, but one that could turn out to be a panacea for you? Each must decide for themselves.
