Medical literature, consultation with physicians, and observation of others.
It is a reasonable guideline, but that's all it is. Some people with depression might need more than 5 mg/day.

If you want to take the lowest effective dosage of Abilify, start low and titrate upwards if necessary. If you are not psychotic (schizophrenia, schizoaffective, bipolar mania), why not start with 2-5 mg/day? You should know within a week if a particular dosage is effective. I don't know whether or not akathisia is dosage-dependent. You might need to take this into consideration when choosing a starting dosage.
