yep, have felt this and similars on occasion as well (total of ~4 years effexor with a 1 year gap). I doubt it has to do with the dosage though. I think it is more the complex action of this drug and interactions with a bunch of endogenous factors - all with different pharmacokinetics and the drug itself clears out relatively fast. I think it can be compared to diabetics having the occasional hypoglycemia.
worst recurring side-effect is the ringing of ears(sort of prelude to real zaps)
side note: I've been a real fool and forgot to stock up before my doctor went on vacation. Went from 150mg to 0, third day, I'm going bananas - head feels like a pinball machine: the zaps are preceded by strong ringing
