This sounds like something that you and your doctor will have to decide.
However, some things to consider are that you may be experiencing social phobia. The criteria (from DSM) are that a patient strongly, repeatedly fears at least one social or perfomance situation that involves facing strangers or being watched by others. The patient patient specifically fears showing anxxiety symptoms or behaving in some other way that will be embarrasing. Here is some good diagnostic information:

It is also important to note that you realize your fear is unreasonable or out of proportion...which may be a sign that your problem is not psychotic in nature.
With that said, I experienced some of the same things you experienced for a while regarding paranoia and thinking people were talking about me. I was diagnosed with Delusional Disorder and given an antipsychotic (Haldol) which I stayed on for about six months.
These are some things to think about and research. I would tell your Dr. how you are feeling and I am sure s/he can help you. If the antipsychotics worked for you before, try them again. Perhaps an antidepressant for your other feelings. Maybe theraphy would be good for you...
One last thing...you realize your social problems are extreme and unreasonable. You also need to realize your feeling of worthlessness is unreasonable. You need to look at things logically and you will see that.
